This sheet is optional and it is last on purpose. It is for the sleeve of money you are genuinely prepared to lose, and it only works if the limits are set before the first position rather than adjusted around whatever you already did.
The readiness check at the bottom is not decoration. If you have not completed thirty days of practice and cannot describe your plan in one paragraph, the honest answer is that this sheet is not for you yet — and skipping ahead is the single most common way this ends badly.
Set these before you start
A cool-off rule is the only one of these that operates on the actual failure mode. Losses do not usually compound because a position was too large; they compound because of what somebody does in the twenty minutes afterwards.

Fill in the exit column before you enter, not after. A planned exit written afterwards is not a plan, it is a memory.
